Steele Choice Insurance, LLC / Logan Steele is a licensed insurance agent/broker. We offer assistance with Medicare plans, including Medicare Advantage and Part D prescription drug plans, and ACA Marketplace health insurance plans.

We are not connected with or endorsed by the U.S. government, the federal Medicare program, CMS, the Health Insurance Marketplace, HealthCare.gov, or any state Marketplace. Our services are provided at no additional cost to you. We may receive compensation from insurance companies when you enroll in a plan.

We do not offer every plan available in your area. Currently we represent 7 organizations which offer 68 products in your area. Please contact Medicare.gov or 1-800-MEDICARE to get information on all of your options.

This Medicare disclaimer is required for third-party marketing organizations that sell Medicare Advantage or Part D plans on behalf of more than one organization, and it must be used verbally before discussing benefits during sales calls, included in electronic communications, prominently displayed on websites, and included in applicable marketing materials.

For ACA Marketplace coverage, Steele Choice Insurance, LLC / Logan Steele acts as a state-licensed, Marketplace-registered agent/broker. We may assist you with eligibility, plan comparisons, applications, plan selection, enrollment, and ongoing service only with your consent.

You are responsible for reviewing and confirming that all information submitted on your Marketplace application is complete and accurate, including household members, income, tax filing status, address, tobacco use, immigration/citizenship information, current coverage, and eligibility for other coverage.

CMS requires agents and brokers to document consumer consent before providing enrollment assistance and to document the consumer's review and confirmation of application accuracy; this documentation must be retained for at least 10 years.

Any personal information you provide will be used only to help evaluate, quote, enroll, and service your insurance coverage, unless you authorize another use. This may include personally identifiable information, health plan information, provider information, prescription drug information, household information, and income information needed for eligibility or enrollment.
